AHCA Financial Consultants:  The AHCA Proof of Financial Ability to Operate form 3100-0009 is designed to meet the specific requirements of 408.810(8), 408.8065(1), and 400.471, Florida Statutes which require the applicant to show anticipated provider revenue and expenditures, the basis for financing anticipated cash-flow requirements of the provider, and an applicant’s access to contingency financing.  AHCA Financial Consultants make sure all schedules must be prepared in accordance with generally accepted accounting principles (GAAP).  A Certified Public Accountant must compile, examine, or apply agreed upon procedures to prospective financial statements, including summaries of significant assumptions and accounting policies.  An applicant will have demonstrated financial ability to operate by completing proof of AHCA Proof of Financial Ability to Operate schedules 1-7 in conformity with the below instructions and GAAP, all relevant rules and statutes, and projections that show assets, credit, and projected revenues meet or exceed projected liabilities and expenses and Independent evidence of sufficient funds for startup, working capital & contingencies.

AHCA Financial Consultants also advise on Change of ownership (CHOW) means an event in which the licensee sells or otherwise transfers its ownership to a different individual or entity as evidenced by a change in federal employer identification number or taxpayer identification number, or an event in which fifty-one percent (51%) or more of the ownership, shares, membership, or controlling interest of a licensee is in any manner transferred or otherwise assigned. A change of ownership application, fees, and all other required forms must be received by the agency at least 60 days prior to the date of change of ownership. Complete and submit the Health Care Clinic Licensing Application, Health Care Licensing Application Addendum, and Proof of Financial Ability to Operate Form.
